Get ready for the season with a sports physical at Nair Urgent Care in Long Beach. We review your health history, perform the required examination, and complete school or sports forms when medically appropriate.
Bring these to your visit
- Your school, camp, league, or team physical form, with the history section completed.
- A photo ID, medication list, and relevant medical records.
- Glasses or contact lenses if you use them.
- A parent or legal guardian for a minor patient. Call ahead with questions about consent requirements.

Please check your organization’s requirements before coming in. Additional evaluation or records may be needed depending on your history and examination. Participation clearance is based on the clinician’s findings.
